This Mission is pleased to assist you to register the birth of your children under

(a) Section 5 (2) of the Citizenship Act and
(b) Section 16 or 24 of the Birth Registration Act.

Please download appropriate forms and applications and comply with the following instructions, incomplete applications may result in delays:

Checklist:

  • Citizenship form (please contact us) to be completed in duplicate by the child's father
  • Registration form (please contact us)- Declaration of birth after 03 months to be completed by the child's father and certified by a JP or a Solicitor on page 2 below the informant's signature.
  • Forms (please contact us) should be sent with separate letters of explanation, addressed to the Registrar General and Secretary, Ministry of Defence, stating the reasons for the inability to declare the birth within three months.
  • Statutory Declaration should be completed by the child's father, in duplicate and certified by a Justice of the Peace or a Solicitor.
  • Forms (please contact us)- for endorsement of child's name - to be completed by the mother or the father depending whose passport the child's/children's name is/are to be endorsed. A concerned letter from the other parent for such endorsement should be enclosed. The application should also be accompanied by two passport size certified photographs of the child/children.
  • Inclusion of Children into Parents’ Passports:  We have been informed by the Department of Immigration & Emigration of Sri Lanka by letter No. OM/02/06(01) dated 24/04/2006 that if children’s photographs are pasted on ‘N’ series passports of parents by Embassies, such passports will be cancelled when they return to Sri Lanka and they will have to obtain new passports by paying the relevant fee. As a result, there could be problems pertaining to visas stamped on those passports. If parents are willing to accept the above-mentioned conditions and repercussions, the Mission is authorized to paste children’s photographs on their passports.
  • Certificate of Evidence of Resident Status (referred to in New Zealand as Certificate of Denial) which is issued by the Department of Immigration in Australia or the Department of Internal Affairs in New Zealand. This can be obtained from either the Head Office or from the Regional Offices. This certificate applies to those who are residing in Australia over two years and in New Zealand over three years on a permanent resident visa. Those who hold dual citizenship should submit their dual citizenship certificate instead of the above certificate.
  • The following original documents should be forwarded with two sets of photocopies with the forms mentioned above.

    (i) Child's local Birth Certificate
    (ii) Parents' Birth Certificate (English translations will not be accepted as originals)
    (iii) Parents' Marriage Certificate (English translations will not be accepted as originals)
    (iv) Parents' passport (two sets of photocopies of the pages with personal data endorsement and renewal etc.)

    On receipt of the above duly completed form and the requested documents, an administrative decision will be made regarding the registration of the birth and the child's particulars will be added to the nominated passport.
    The application in respect of Citizenship registration will be sent to the Sri Lankan Defence Ministry and upon receipt of the prescribed certificate and due registration in the Mission it will be forwarded to the parent of the child. You are advised to keep this Mission posted about your current address and your telephone number, should you move address before receiving the registration certificate.

  • Citizenship Certificate - A$ 50.00
  • Late Registration of birth (beyond 1 year since birth) A$ 12.00 per year
  • Endorsement of child's name in either passport - A$ 10.00
