Amendment to Sri Lanka Passports - endorsements/Deletion and/or Alteration of child222s name:
Checklist:
- Current Sri Lanka passport
- Child222s Birth Certificate/Citizenship Certificate issued by the Sri Lankan authorities (original and a photocopy)
- A letter of consent signed by both parents :
- at the Sri Lanka High Commission , in Canberra or at the Consulate General in
- Sydney
- in the presence of a Honorary Consul for Sri Lanka in ( Australia - Melbourne,
- Adelaide, Perth , NZ-Wellington , Fiji)
- in the presence of a Justice of the Peace/Notary Public and thereafter obtaining
- certification/authentication of the signature of the Justice of the Peace/Notary Public by the Department of Foreign Affairs and Trade.
- Three certified photographs by a Justice of the Peace, Practicing Medical practitioner or Solicitor
- If a name of a child who is born outside Sri Lanka is to be endorsed in mother's/father's passport, the child's birth must first be registered in this Mission before the endorsement is made.
